The Asheville Hospice and Palliative Medicine Fellowship

Qualified physicians who wish to develop, enhance and refine their knowledge and skills in hospice and palliative medicine are invited to apply for the salaried, ACGME-accredited Asheville Hospice and Palliative Medicine Fellowship, in affiliation with the Mountain Area Health Education Center and the UNC-Chapel Hill School of Medicine.

Fellows in the innovative MAHEC post-graduate training program function as fully integrated members of interdisciplinary teams, with core rotations providing experience at Four Seasons Compassion for Life, an independent non-profit, Flat Rock, and CarePartners, a division of Mission Hospitals, Asheville. The two established, respected hospice and palliative medicine providers offer direct experience of current care models.

The Four Seasons Community Palliative Care Model

In May 2014, Four Seasons Compassion for Life was chosen to receive a grant award of $9.5 million from the Center for Medicare and Medicaid Innovation, a program of the federal Department of Health and Human Services, to implement health care reform over the next three years through its innovative community palliative care (CPC) model.

During rotations with Four Seasons, Fellows in the program will gain first-hand knowledge of the CPC model, widely expected to establish the national benchmark standard for palliative care.

The Four Seasons CPC model removes barriers to patient care due to location, socio-economics, ethnicity or minority status, and fragility of medical condition. The model features interdisciplinary collaboration and the integration of palliative care into the health care system, continuity of care across transitions, and longitudinal, individualized support for patients and families – with the ultimate goals of increased quality of care, decreased hospital readmissions, and lower costs.

Core Rotations

Physicians in core rotations obtain equal inpatient and outpatient experience with Four Seasons and CarePartners.

Intensive in-patient hospice care experience includes rotations at Four Seasons’ 19-bed Elizabeth House and CarePartners’ 27-bed Solace unit. Physicians will provide palliative care consultations for discussions of patient goals and symptom management throughout Mission Hospital, an 800-bed regional hospital.

Palliative care experience includes long-term care, in-home, and outpatient clinic settings. Additionally, Fellows collaborate with team members across disciplines through the delivery of hospice home care. Our Fellows serve patients of diverse socio-economic backgrounds ranging from Main Street to mountaintops and factories to financiers – with service delivery in private homes and skilled care locations in both urban and rural settings.

Supervision and Mentoring

Across settings, Fellows are supervised and mentored by experienced hospice and palliative medicine physicians who understand that hospice and palliative medicine is intellectually challenging and emotionally and spiritually demanding. As a result, personal growth and self-care are emphasized.

Additionally, Fellows can meet regularly with a designated hospice chaplain and a bereavement counselor, to discuss issues, explore attitudes, and develop skills needed to sustain a successful tenure in hospice and palliative medicine.

Curriculum and Electives

Community faculty supplements the regular curriculum with specialty rotations and electives in a broad range of specialties.

  • Weekly conferences cover topics ranging from core clinical knowledge, skills and training in practice management, research skills, self-care and spirituality, and media.
  • Fellows may choose to customize their education and focus additional time in a core area of particular interest for a period of one month.
  • The Palliative Care Book Club explores the landmark, current and emerging literature related to care-giving during life-limiting illness or at end of life.
